School Nurse

- My name is Mrs. Roxanne Shimek.

- I am a Registered Nurse, licensed to practice in the State of Indiana.

- I travel between Calumet High School and Lake Ridge Middle School.

- My schedule is as follows:  Monday, Wednesday morning, and Friday at LRMS.  Tuesday, Wednesday afternoon, and Thursday at CHS.

- Parents - It is vitally important that you inform me if your child is diagnosed with any disease that could be a concern during the school day, or which may affect their learning.  Please contact me asap should this be the case.   

-School board policy requires that students not carry any medicines with them with the exception of emergency inhalers and epi-pens.  Please inform me if your student has need of these medicines, or if you need to leave other medicines in my office for your student to use during the school day.   

-In addition, please inform me immediately if your student has an illness that is contagious such as chicken pox, H1N1, MRSA, etc.  We need to keep the rest of the school population safe and informed as appropriate.  

-Please be aware that the state of Indiana has new immunization requirements as of the 2010-2011 school year. Students who are not in compliance will  be excluded from school beginning 20 days from the start of the school year.  

- The Lake Ridge School Based Health Clinic at Calumet High School is available to all Lake Ridge students for illnesses, school physicals, and behavioral health needs.  A referral from one of the school nurses is required before being seen during school hours.  The clinic also has hours after school for the community as a whole. Call 880-1190 for more information.

Updating Phone Numbers!!!

During an emergency, it is extremely important that the nurse be able to reach the parents of a sick or injured student. PLEASE REMEMBER TO UPDATE ANY CHANGE IN HOME OR WORK PHONE NUMBERS.

The emergency number should be a friend or relative WHO WOULD BE AVAILABLE TO TRANSPORT THE STUDENT HOME if the parents are not available.

Parents who are going out of town should send a note or call the school with information regarding who will be responsible for the student during the parent's absence.
